The Health Promotion and Chronic Disease Prevention (HPCDP) Section of the Oregon Health Authority (OHA) is contracting with the ORI CES to evaluate HPCDP’s work with the three Regional Health Equity Coalitions (RHECs) funded since 2011 to identify successes and lessons learned related to two topics: A) Integration of tobacco and chronic disease data into local equity planning, and B) Engagement in public health related policy, system, and environmental change into community conversations and plans that seek to increase health equity.

Information collected will help inform HPCDP’s quality improvement efforts to strengthen the state public health system’s infrastructure to support RHECs in contributing to community health policy initiatives.
